Family Model Provider – Middle TennesseeMid-South Care Services | Contract OpportunityPosition OverviewMid-South Care Services is seeking compassionate, dependable individuals and families throughout Middle Tennessee interested in becoming Family Model Providers.Family Model Providers welcome adults with intellectual, developmental, or physical disabilities into their homes, providing a safe, supportive environment while promoting independence, dignity, and quality of life.Key ResponsibilitiesProvide a safe, clean, and welcoming home environment.Assist with daily living activities, personal care, and individual needs.Promote independence, community involvement, and meaningful relationships.Follow each individual's Person-Centered Support Plan (PCSP).Provide transportation to appointments and community activities.Maintain accurate documentation and communicate with Program Coordinators.Ensure individual health, safety, rights, and regulatory compliance.Qualifications & RequirementsMust be at least 18 years old.Have a safe, stable home with appropriate accommodations.Maintain current Homeowners or Renters Insurance.Maintain current Automobile Insurance and reliable transportation.Complete all required Relias Training.Obtain and maintain Medication Administration Certification.Successfully complete required background checks and screenings.Complete required home inspections and ongoing monitoring.Maintain all required certifications and compliance documentation.Demonstrate compassion, dependability, and strong communication skills.Previous caregiving experience preferred but not required.Why Become a Family Model Provider?Receive compensation for providing approved residential support.Make a meaningful difference from your own home.Help individuals achieve independence and personal goals.Receive ongoing training, guidance, and support.Join Our Family Model Program!If you have a welcoming home and a passion for helping others, we encourage you to apply!Mid-South Care Services is committed to providing quality, person-centered services throughout Tennessee.
